YOUR LIFE EVENTS

When you enroll as a new hire, or during the annual open enrollment period, many of your benefit selections remain in effect for the entire calendar plan year. However, you may make a change to your benefits if you or your family experience a change that qualifies as a “Life Event.” In this section, we’ve outlined common life events, including the things you need to consider and actions you need to take to make sure any benefit changes go smoothly.

SEPARATION OF EMPLOYMENT

When you leave the company, you may have questions about pay, benefits and what you need to do. Detailed benefit information will be mailed to your home. The following information is provided to help guide you through the transition.

If you plan to resign from the company, whether you are pursuing a new opportunity or retiring, you should provide verbal and written notification to your supervisor at least two weeks before your last day. Please ensure your mailing address is current, or enter your new mailing address if changing, to be sure you receive all post-employment benefits and payroll information.

Your last paycheck

You will receive your last paycheck in the normal payroll cycle after your last day of employment. Normal benefit deductions will come out of your last paycheck.

Continuing benefits

Your core health benefits (Medical, Dental and Vision) end on the last day of the month your employment ends. All other benefits end on your last day of employment. When your benefits end, you may choose to continue health benefits under COBRA continuation, while other benefits may be converted to personal policies by contacting the benefit carrier directly. Notifications on extending your benefits will be mailed to you. Please allow two weeks after your termination is processed to receive this information.
