ELIGIBILITY STATUS

Hourly outfitters

Hourly outfitters who will meet their 90-day waiting period are eligible to enroll. You must enroll within 30 days of that date. (The benefit effective date will be on the 91st day.)

Regular part-time outfitters

Regular part-time outfitters who will meet their 90-day waiting period are eligible to enroll in dental, vision and voluntary benefit plans. (PACE, temporary and seasonal outfitters are not eligible.)

Salaried outfitters

You are eligible for benefits on your date of hire. You must enroll within 30 days of your hire date. (The benefit effective date will be on the date of hire.)

Dependents

You may enroll your eligible dependents in medical, dental and/or vision benefits, but you must have proof of eligibility to add them to your coverage. Eligible dependents include your:

  • Legal spouse.
  • Child(ren) up to age 26, regardless of marital or student status.
  • Unmarried children of any age who are incapable of supporting themselves due to a mental or physical disability and who are totally dependent on you.

Don’t wait until the last minute to enroll!

Make sure you’re covered if you need care by enrolling in your benefits as soon as you’re eligible or prior to your effective date.
